2025-2026 Health Forms

All health forms, except those that need to be filled out by a medical care provider, are fillable PDFs that can be submitted online. Forms A (Part 2), B, E, F, and G will need to be printed, as they require signatures from your child’s medical care provider.

All forms must be received by the office by Monday, August 18. Medications must be received by the office by the first day of school on September 2. Please reach out to office@onenessfamily.org if you have any questions regarding these forms.

Please note the following requirements for student health forms:

New Students

  • All new students must submit forms A-D.
  • Students with allergies, asthma, or required medication to be administered during the school day are required to submit forms E-G.

Allergies, Asthma & Medication

Students with allergies (Form F), asthma (Form G), or those that require medication to be administered during the school day must have (Form E) signed by a doctor with any accompanying medication in its original packaging. There should be one completed and signed copy of Form E for each medication that is required.

Returning Students

  • Returning students in Little Stars and the Children’s Peace Garden must resubmit forms A-D. 
  • Returning students in Grades 1-5 must resubmit form D.
  • Returning students in Grades 6-12 must resubmit forms D and H.
  • All returning students with allergies, asthma, or required medication to be administered during the school day are required to resubmit forms E-G.
