All forms must be received by the office by Monday, August 10. Please note that Oneness-Family School is required by law to have necessary health forms for all students on campus.
All health forms, except those that need to be filled out by a medical care provider, are fillable PDFs that can be submitted online. Forms A (Part 2), B, E, F, G, and I will need to be printed, as they require signatures from your child’s medical care provider.
Medications must be received by the office by the first day of school on August 31. Please reach out to office@onenessfamily.org if you have any questions regarding these forms.

For New Families
Required
If Applicable
Students with allergies (Form F), asthma (Form G), seizures (Form I), or those who require medication to be administered during the school day must have Form E signed by a doctor with any accompanying medication in its original packaging. There should be one completed and signed copy of Form E for each medication that is required.
Additionally, students in middle and high school should complete Form H (field trip authorization) annually.

**Form A and Form B are required for resubmission if any medical changes or updates to vaccinations have been made.
For Returning Little Stars & Children’s Peace Garden Families
Required
If Applicable
