Medical Authorization Form Due – Costa Rica Spring Trip

Dear Oneness Family Community,

As we prepare for our upcoming Spring Trip to Costa Rica from May 11–May 15, this is a reminder that the Medical Authorization Form is due by Friday, May 1st.

Attached below is the Medication Authorization Form (Form E).

Please review the following important deadlines and requirements:

Medication Authorization Form (Form E):
If your student requires medication while on the trip, this form must be completed, signed by a physician, and returned to us by Friday, May 1st.

Medication Submission Guidelines:
For any medications your student will need during the trip (that they do not typically take during school hours), please ensure they are brought to the Administration office by Friday, May 1st, and meet the following requirements:

  • Must be in the original container
  • Must have a pharmacist label with the student’s name
  • Must be in the original packaging

All medications must be turned in by the deadline to ensure proper handling and compliance with school policies.

Our priority is to ensure we are fully prepared to support all students in case of an emergency and to remain in compliance with medication guidelines.
